Player Registration Information

1. Physical Card - for all new athletes and returning athletes whose Physical from last season is dated previous to April 1, 2019
2. Alternate Year Card - for those whose Physical Card is up-to-date. Not needed if they are turning in a new Physical Card.
3. Risk of Injury/Athletic Permit
4. Covid Release
5. Concussion Form
6. Activities Code of Conduct
7. Emergency Card

Westosha/Central High School Forms

Athletes should register at the following site.

Physicals are also needed. 

"Physical examinations taken April 1 and thereafter are valid for the following two school years; physical examinations taken prior to April 1 are valid only for the remainder of that school year and the following school year.  This means any physical prior to April 1, 2019 will not be accepted for the 2020-2021 school year.  "

  • Central students also need to fill out the Consent to Treat form.

Wilmot High School Forms

Athletes should register at the following site.

Step 1: Once you have the physical form completed you may either upload it with your registration or bring it to Mrs. Johnson or Mr. Christiansen in the student services office.

  • Physicals after 4/1/2019 are good until 8/1/2021
  • Physicals after 4/1/2020 are good until 8/1/2022

Step 2: Complete all sections of the Registration Form. 

Step 3: A $40.00 payment by Credit Card or Cash/Check per Athlete is required for the school year.  

Step 4: Wilmot students also need to fill out the Consent to Treat

Player Fees

The fee for all Thunder hockey players (in addition to school fees) is $900 per player.  Additionally, each player is required to purchase the apparel package for $300.  Players are also required to wear a maroon helmet,  and black gloves. Information on this fee will be provided shortly.
